WAIVER & RELEASE OF LIABILITY AGREEMENT
I realize that there are dangers inherent in glassblowing, glass working, and other related activities, and that mortal or serious personal injuries and property damage, including (but not limited to) physical effort, seizures, cuts, burns, and/or exposure to heat and fire, may occur from my participation in such activities.
I hereby assume full responsibility for any and all risk of personal injury, death, and property damage due to the negligence and or fault of the parties released by this document, and/or due to the condition of the premises on which the activities will take place, whether such negligence, fault, and/or the condition of the premises is present at the signing of this agreement or takes place in the future.
On my own behalf and on behalf of my heirs, personal representatives, and assigns, I hereby release
P.T. Horizon Glassworks and all of their officers, directors, members, managers, partners, employees, contractors, and volunteers (collectively, “the parties” released by this document) from any and all claims, demands, actions, rights of action, or other legal rights to claim compensation of any kind for any loss or injury which I may sustain as a result of my own or their negligence or fault, or the condition of the premises, or any other cause whatsoever, whether loss or injury occurs while participating in glass blowing, or any other activities on the premises including going to, or coming from such activity and location.
Further, I agree to indemnify and hold all parties released by this waiver/release harmless from any such claims or demands whatsoever. Expressly agree that this waiver and release agreement is intended to be as broad and inclusive as permitted by the laws of Bali & Indonesia and of any other state or country wherein such activities may occur, and that if any portion thereof is held invalid, the remainder hereof shall continue in full force and effect.
This Agreement contains and embodies the entire agreement and understandings between the parties concerning the subject matter hereof.

Optional Photo & Video Release Form
I hereby grant permission to P.T. Horizon Glassworks ET AL, the rights of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video tape ☐ without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published, modified or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording. I also understand that this material may be used in diverse educational settings within an unrestricted geographic or metaverse/universe area.
Photographic, audio or video recordings may be used for ANY USE which may include but is not limited to:

Online/Internet Videos;

Media;

News (Press);
By signing this release, I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

Cancellations and Refunds
If you cancel 48 hours or more in advance you will receive a full refund. If you cancel within 48 hours of the course you will receive a 50% refund. If we cancel at any time you will have the opportunity to reschedule or get a full refund.

Cancellations can occur for many reasons such as minimum paticipants not reached, power outage, unsafe driving weather, staff illness, death of a village member, unplanned ceremonies, driving accidents, and severe traffic jams can all cause staff absence.
